Five things

Let me ask you folks a hypothetical question out of sheer curiosity. If you were going into the woods for say a week, and you could only take five items with you. What would they be?

Dang it. If I had to choose I would take a larger knife at this point.

Hammock kit. Includes tarp and bug net all straps and tie outs.
10/22. with 200 rounds.
mess kit. includes folding wood stove and a brasslight alcohol burner. small compass, fire kit, dawn dish soap, and extra virgin olive oil.
saw kit. includes a multi position handle, several wood blades, and a metal one.
Pulk. It can easily be pulled on the ground and help handle firewood after processing.
pessimist complain about the wind. optimist expect the wind to change. realist adjusts the sails.

I would need some thing to cover these needs.

If I was going into the woods on purpose...... and I knew it was for a week(5 days or 7 days)....... I wouldn't go with anything less than my full array of gear depending on area, time of year, what my activity would be, and weather conditions.

Having done such things on purpose before (check out the "Pot and Machete" challenge)
I can tell you I would not be going to the woods with a simple 5 items unless it was associated with another of these idiotic challenges we do.
That said, I like XJ35's answer

Assuming I'm not counting my clothing and am not carrying EDC stuff:

FAK (counting as one item)
Tarp (or sleeping bag depending upon weather)
Spark Stick
Pot
Knife or Axe

This list takes care of my first three priorities of health, shelter and water:
FAK - First Aid/Health emergencies
Tarp if I'm expecting windy/wet conditions (or sleeping bag if I'm expecting cold dry conditions)
Spark Stick to provide fire to augment shelter and to provide for clean water
Pot - to provide potable water by boiling
Knife if I'm expecting moderate to warm and dry condtions or an Axe if expecting cold dry or wet conditions

I like lists like these as they demonstrate different ways of thinking about priorities.
You can see, I basically looked at the rule of threes and covered the gear needed for up to three weeks.
The lack of food would of course be uncomfortable, but doable. I really don't think I could see myself trialling this out voluntarily though.
The longest I've been out with minimal kit was a 24 hour test of a PSK.
